Base salary: $105,000 – $190,000+

This full-time Remote Registered Nurse Care Manager opening for a U.S.-based remote team is with a confidential virtual care-management team. The successful candidate will assess member needs, coordinate care, and help patients follow practical plans across providers and care settings. About the Role

This position gives an experienced nurse ownership of longitudinal patient support in a fully remote care model. Day to day, the person in this position will assess member needs, coordinate care, and help patients follow practical plans across providers and care settings. The employer will confirm permitted work locations, core collaboration hours, travel, equipment, and performance expectations before formal interviews begin. During interviews, candidates can ask directly about reporting lines, workload, performance measures, and the reason for the opening.

Responsibilities

  • Complete telephone or video assessments and identify medical, behavioral, social, and access needs.
  • Build and update care plans with patients, caregivers, providers, and community resources.
  • Support medication, appointment, discharge, and referral follow-through.
  • Escalate urgent concerns and document outreach, decisions, barriers, and outcomes accurately.
  • Build dependable working relationships with patients, providers, and support staff while meeting documentation and safety standards.

Required Qualifications

  • Active Registered Nurse license in an approved state; compact licensure may be required.
  • Clinical nursing experience plus care-management, utilization, transition-of-care, or population-health experience.
  • Confident telephone assessment, patient education, and interdisciplinary communication.
  • Reliable remote documentation habits and a private work environment.
  • Demonstrated ownership of complex work, sound delegation, and clear communication with clients or leadership.

Preferred Qualifications

  • Experience in a comparable care setting or patient population.
  • Current role-specific certifications and familiarity with modern clinical documentation systems.
  • A record of improving results, developing people, or expanding responsibility over time.
